For a very reasonable annual premium of $400 per year, plus a one-time sign-up fee of $150, patients of Summit Dentist are entitled to receive two checkups per year including comprehensive and/or periodic oral evaluations, one x-ray series per year (either an intra-oral x-ray series or a smaller checkup series, depending on what the patient requires), and two thorough teeth cleanings per year-the actual value of all of which is $825. Additionally, signing up with the plan also carries with it a 15% discount off the usual and customary fees on diagnostic and preventative procedures. This discount also applies to other basic dental procedures, such as deep cleanings, root canals, and white composite fillings, as well as major procedures, such as extractions, crowns, bridges, implants, and orthodontics.
Terms and Conditions
The annual premium is paid at the time of arrangement and is valid for one full calendar year beginning on that day. This plan may not be combined with any other insurance plan(s), discount plan(s), or coupons. Patients are responsible for the payment of discounted fees at the time of services rendered. Patients must renew the plan annually to avoid paying the sign-up fee more than once.
To enroll in this plan, all patients have to do is fill out a simple form (which can be obtained on-site at Summit Dentist's office on the first floor of 137 Summit Avenue in Summit, New Jersey) and return it with a signature, and either credit card information or a check attached. Checks should be made payable to Summit Dentist.
